Abstract

1984년 4월부터 1989년 5월까지 서울대학교병원 치과 진료부와 원자력 병원 치과를 내원
하고 부비동 전산화 단층 촬영을 시행한 25례의 상악동 악성종양을 관찰하여 다음과 같은
결론을 얻었다.
1. 평균 연령은 54세 였으며 남자 18례, 여자가 7례, 남녀 성비는 2.6 : 1로 남자에서 많았
다.
2. 가장 흔한 암종은 편평상피암으로 19례에서 관찰되었으며 선양낭암이 2례, 그리고 악성
섬유성 조직구종, 점액 상피성 종양, 조직구성 임파종, 미확인된 악성 종양이 각 1례였다.
3. CT소견은 상악동내 음영증가가 21례(84%), 연조직 종괴 23례(92%), 낮은음영 11례
(44%), 공기음영 6례(24%), 골경화 1례(4%), 골파괴 23례 (92%), 지방층 소실 19례(76%),
골전위 8례(32%)였다.
4. 상악동 악성 종양 환자에 있어서 CT촬영은 종양의 비강, 사골동, 안와 측두하와, 익구
개와, 협측피부, 익상와, 익상근, 두 개강등의 인접조직으로의 침범 여부 및 림프절 전이를
평가하는데 도움을 주었다.
5. TNM system에 의한 병기 분류는 Stage Ⅲ, Stage Ⅳ가 24례(96%)를 차지하였다.
6. 골 소견은 골파괴, 골전위, 골경화를 보였고 상악동 내벽의 파괴가 23례(92%)로 가장
많았다.
7. 종양의 성장 유형은 25례중 18례가 침윤성 성장을 보였고 편평상피암은 주로 침윤성
성장을 보였다(P<0.05).

CT findings of proven 25 malignant tumors of the maxillary sinus were
retrospectively analyzed to be of help in the diagnosis and treatment.
The results were as follows :
1. Average age was 54 years old, and eighteen were males and seven were females
with a ratio of 2.6:1.
2. The most common histopathologic feature was squamous cell carcinoma (19 cases)
and others were two cases of adenoid cystic carcinoma, one case of malignant fibrous
histiocytoma, mucoepidermoid tumor, histiocytic lymphoma, unidentified malignant turner.
3. CT findings were sinus opacification (4%), soft tissue mass (92%), low densities
within soft tissue mass (44%), air densities within soft tissue mass (24%), osteosclerosis
(4%), bone destruction (92%), bone displacement (32%), fat plane obliteration (76%).
4. CT in the malignant maxillary sinus tumors approved the value in evaluation of
tumor extension to nasal cavity, ethmoid sinus, orbit, infratemporal fossa, pterygopalatine
fossa, pterygoid fossa, pterygoid muscle, cheek skin and intracranial cavity.
5. Twenty four cases (96%) were stage Ⅲ, stage Ⅳ according to AJCC TNM
classification.
6. Bone findings were destruction, displacement, sclerosis and most frequent site of
bone destruction was the medial wall of the antrum (92%).
7. Tumor growth pattern showed destructive pattern in 18 cases (72%), and squamous
cell carcinoma showed destructive pattern. (P<0.05)
